Whole Person Care is our commitment to seeing people as people, not problems to solve. It’s a long-term, hands-on approach that addresses the entire picture of a person’s life. Instead of focusing on just one need, we help untangle the layers: housing, health, income, safety, transportation, and more.

What does that look like?

Initial Contact
When someone reaches out, they’re paired with a Care Navigator, a consistent partner who sticks with them through the journey. This isn’t a handoff or a one-time service. It’s a relationship.

Needs Assessment
We take time to understand the full story. Clients work with their navigator to identify what's getting in the way, whether it’s childcare, substance use, lack of ID, or health care access. We don’t assume. We ask.

Goal Setting
From there, we map out a personalized plan. It's built with the client, not for them. It includes realistic goals, timelines, and priorities that make sense for where they are right now—not where we want them to be.

Integrated Support
We don’t expect anyone to solve every problem alone. Navigators coordinate services across HopeSource and with community partners, connecting people to mental health care, housing assistance, employment programs, utility help, transportation, legal aid, and more.

Ongoing Partnership
Whole Person Care means walking alongside someone from crisis to stability to thriving however long that takes. It’s not about checking boxes. It’s about building momentum and staying with them until they’re confident on their own.
