The Right Support at the Right Time: Jennifer’s Story

Since early 2021, Jennifer, a local senior in Kittitas County has quietly been navigating a challenge so many know all too well, trying to stay afloat on a fixed income while health needs steadily grow. 

At first, it felt overwhelming. Rent, medical bills, daily living, it all began to blur into a constant worry. That’s when she connected with a HopeSource care navigator, a relationship that would change everything. 

Through the Senior Rent Program, something as simple, but life-changing, as stable, affordable housing became possible again. It meant more than just keeping a roof over her head; it meant independence. Familiar surroundings and dignity. 

But the support didn’t stop there. 

Because of the wraparound care provided, Jennifer had continued connection with a care navigator who could check in on her to ensure all their needs are being met. In those check-ins she began to open up and share other needs. Things like in-home caregiving support, medical and mental health providers, and energy assistance resources, all of which the care navigation team could provide . With those supports in place, she's been able to remain stable, housed, and independent far longer than she thought would be possible. 

Now, as her health needs continue to grow, she is preparing for the transition into a nursing facility. But this time, it looks different. This time, Jennifer is moving forward with stability, dignity, and a team already in place to support her every step of the way.  Her journey is a powerful reminder that the right support at the right time can be all it takes to ensure our seniors are well cared for in the community.
